Functional Life Skills Assessment
Goal: Develop a clear understanding of life skills strengths and weaknesses in order to provide families with specific targets for life skills programming.
These assessments may benefit any youth or young adult who has gaps within their daily routines and would benefit from targeted support to increase independence. At WonderTree, we use standardized assessment tools to explore the client’s life skills, complete interviews with the client and caregivers, and read relevant documentation. The most important factor in creating a plan is to focus on the individual. Their strengths, needs, and preferences will be vital to a successful support plan.
Examples of areas of functional life skills that can be assessed and targeted for growth:

Home living skills
Once the assessment is completed, our WonderTree Developmental Service Worker can then offer sessions targeting the skill areas discussed with the client, family, and respite workers, as needed. Check out our Life Skills Support.
What Does a Functional Life Skills Assessment involve?

Intake Meeting
Meeting with family/caregivers, client, and our DSW, as well as a review of relevant data (e.g., prior assessments, relevant medical documentation).

Assessment
Completion of standardized questionnaires by family/caregivers, the client, and school staff (if consent provided)

Comprehensive Feedback
The feedback session is directly between the client and/or caregivers, and the DSW. The school or community partners are also welcome to join with consent of the youth/parents. The DSW will explain the results of the assessment, highlighting the areas of strength and need, and providing recommendations and next steps.

Report
The report will include the results of the questionnaires, as well as programming recommendations tailored to the individual for home, school, and the community.
Who would benefit from a Functional Life Skills Assessment?
WonderTree life skills assessments may be beneficial for youth with mild to moderate intellectual disabilities, autistic youth, individuals with borderline intelligence, or youth with ADHD who struggle with daily living skills. We would be happy to speak about the usefulness of this type of assessment for your child if they do not fall in the above categories.
Fee: $500
These assessments are supervised by a registered psychologist. Psychological services are not covered by OHIP, but may be covered under your extended health insurance benefits (here is a list of questions you can ask). Check with your insurance provider for details.
